Why Hamstring Strains Keep Coming Back

Hamstring strains are one of the most common injuries in sprinting, football, soccer, and any sport built on fast acceleration. They're also uniquely prone to repeating. Across studies of elite athletes, recurrence rates for hamstring strain injury range from roughly 14% to as high as 63% within the same or following season, and nearly a third of reinjuries happen within the first year of returning to sport (Martin et al., 2022).

Recurrences aren't just common — they're often worse than the original injury. A second strain typically means more torn tissue, a longer layoff, and a longer road back. That's the real cost of returning to sport too early or rehabbing the injury incompletely.

The Reinjury Window Is Real

Most repeat strains happen fast. In elite Australian football players, over half of all recurrent hamstring injuries occurred within two months of the index injury (Martin et al., 2022). That tight window points to a specific failure: the muscle was cleared to return before it had rebuilt the strength and length it needed to absorb high-speed loading again.

What's Actually Causing the Cycle

A hamstring strain almost always happens at the same moment in a sprint stride — late swing phase, right before the foot strikes the ground, when the hamstring is lengthening and contracting at the same time to decelerate the leg. That's an eccentric contraction, and it's the single hardest thing to ask a muscle to do well.

Incomplete Eccentric Strength

If rehab stops once pain resolves and normal walking feels fine, the muscle often hasn't been rebuilt to handle that eccentric demand. It can feel "healed" at everyday speeds and still fail the moment you sprint again. This is the single biggest driver of the reinjury cycle.

Scar Tissue and Fascial Restriction

Strained muscle fibers heal with scar tissue, which is stiffer and less elastic than healthy muscle. Left untreated, that scar tissue changes how force transmits through the muscle-tendon unit and creates a weak point that's more likely to tear again under the same load that caused the first injury.

Skipping the Return-to-Sprint Progression

Returning straight from jogging to full-speed sprinting — without a graded build through strides, accelerations, and top-speed efforts — is one of the most common rehab shortcuts. The hamstring needs to be reintroduced to high-speed eccentric loading gradually, not all at once on game day.

Breaking the Cycle: What Effective Treatment Looks Like

Clinical practice guidelines from the American Physical Therapy Association's sports and orthopaedic sections identify eccentric strengthening — performed with the hamstring in a lengthened position — as one of the most well-supported interventions for both treating hamstring strains and preventing recurrence (Martin et al., 2022). That means the rehab exercise has to load the muscle in the same stretched, lengthened position where the original tear occurred, not just build general strength.

Where Dry Needling Fits In

For the scar tissue and residual tightness that often linger after a strain, dry needling is a targeted tool. A 2025 systematic review of dry needling in sports and sport recovery found the strongest and most consistent effects were on pain and muscle stiffness, particularly in lower-limb muscle groups, when used alongside an active rehab program (Kużdżał et al., 2025).

At-Home Steps to Start Today

  1. Nordic hamstring curls (assisted). Kneel with your ankles anchored, and slowly lower your torso forward under control, catching yourself with your hands before you fall. This is the gold-standard eccentric exercise for hamstring resilience. Start with 2–3 sets of 3–5 controlled reps.

  1. Single-leg Romanian deadlift. Standing on one leg, hinge forward at the hips while reaching toward the floor, keeping your back flat and the standing knee soft. This trains the hamstring in the lengthened position where strains occur. Do 8–10 reps per side.

  1. Glute bridge march. Bridge your hips up, then alternate lifting one foot at a time without letting your hips drop. This builds the glute strength that should be sharing the sprinting load with your hamstring. Do 10 reps per side.

  1. Progressive stride-outs. Once cleared for light running, build from a 50% effort jog to 70%, 80%, and 90% efforts across separate sessions before returning to full sprinting. Never jump straight from jogging to game speed.

Consistency matters more than intensity here — two or three short sessions a week, done for the six to eight weeks after a strain, is what actually changes the tissue's capacity.
